35. Parent Questionnaire
This is a fantastic form that helps you make links with the parents, whose children you care for, and offer opportunities for them to share information on their children.
You are able to ask about specific areas using the drop down menu's, and then choose the questions that you ask. The parent can then either tick whether or not they are happy with this certain area, and also have the opportunity to expand on their answer with a written statement in the box provided. |
All of the drop down boxes can be edited, so the topic or questions asked can be specific to the information you would like to ask. The feedback that you receive from these questionnaires can then be linked to your reflective practise, by identifying areas that you can change and implement into your setting. This can then be reviewed to see how effective it was, and if any further changes are required. The same questionnaire could then be sent to the parents at a later date, to gain feedback on the changes that you made. |
